WDO Forms

To install one or more of these forms below into a current version 12 or later 3D Inspection System software:

1. Download the desired file(s) and copy the file into your 3D software "Data" location, usually My Documents3D Inspection System 11 folder. (To verify your specific data location, use the 3D Inspection System Report Writer, Preferences > Preferences, File Locations tab.)

2. Open your 3D software. Generally, forms will automatically install so they are available to create new inspections. However, if they do not install automatically, you may use the Report Writer File > Install a set of forms" to select and open (or double-click) the desired form installer .3DF file you downloaded to install it. Note: If you are updating or replacing an existing form group already installed, you may choose to overwrite them. Overwriting existing forms may undo custom changes you previously made to those forms.

Once installed, you may use the new form group to create new inspections.

NPMA-33

 

Description

THIS DOWNLOAD was the now obsolete NPMA-33 - Wood Destroying Inspection Report (9/01/04) originally developed with the National Pest Management Association and made available through HUD in the past as a publicly available form at that time. THIS FORM IS EXPIRED AND IS NO LONGER SUITABLE FOR PRODUCING NEW REPORTS IN 3D.

NPMA since revised the NPMA-33 form and asserts copyright ownership of its current format which is adopted for use in many states . NPMA informed us that software companies wanting to distribute the form within their software would need to pay an exorbitant annual licensing fee. Otherwise inspectors who wish to update their form for electronic use should obtain appropriate licensing from them to reproduce and use the form in their software.

NPMA now requires payment or licensing for use of the NPMA-33 form in any format.

Any questions about the legality of a private organization imposing fees to use a form required by various government agencies should be directed to your local state representatives. Some states have opted to create their own alternative approved pest forms that do not require payment for licensing and use, and some of those alternative state forms may be available for download and use within 3D. Other states or organizations may only currently accept the NPMA-33.

NPMA-99A

 

Description

HUD NPMA-99A (8/2008 exp. 09-30-2022)
A Subterranean Termite Protection Builder's Guarantee. This form is submitted for proposed (new) construction cases when prevention of subterranean termite infestation is specified by the builder or required by the lender, the architect, HUD, FHA or VA. This form is completed by the builder.

NPMA-99B

New Construction Subterranean Termite Service Record. This report is submitted for informational purposes to the builder on proposed (new) construction cases when treatment for prevention of subterranean termite infestation is specified by the builder, architect, or required by the lender, the architect, HUD, FHA or VA. Description

**Updated 2013** California Wood Destroying Pests and Organisms Report 43M-41, designed so comments can grow and print narratively. The updated form design was released to replace the prior limited graphical form which is no longer supported, and to comply with Pest Control Act July 2012.

Estimates: Note that to use the special Estimates field and feature, you must use Preferences to turn on Estimate feature. Enter the actual Estimate in the lower pane Current Line tab for each Estimates field throughout the report (do not enter the estimate itself in the upper field area). To optionally ALSO print this amount in report body below Recommendations, check the Include box above in the Estimates fields. Use File menu to automatically generate separate Estimate sheet with totals, for inclusion as a PDF or Excel worksheet.

Proposal: Use File > Page Setup while a report is open to access the master WDO Proposal that automatically attaches and prints at the end of the report. Edit the specific proposal attached to the report by double-clicking the _index.w3d in the lower Attachments pane of the report to add section totals or other specifics as desired.

Diagram: Adjust Diagram scale percent setting in page setup to make schematic larger/smaller to fit on first/second page as desired. The included Schematic features may be used, or one may insert a standard graphic from another program for inclusion into the report.
